Why Century Oak Care Center?

Century Oak Care Center opened in March 1987 because of the increased demand for quality nursing home facilities.  The owners of COCC decided from the beginning that there was a need in the market for a more upscale choice with a family and community feel, and set out to create a nursing home facility where they themselves would be comfortable having their own family members reside. 

A Unique Facility
The facility was specially designed to give this community feeling from the time people set foot through the front doors.  Just inside the main entrance, people find a large, open lounge and dining room that is a comfortable and welcoming gathering place.  It is immediately evident that Century Oak is very different from other long term facilities.

Century Oak also features three nurses’ stations, four lounges, two central bath areas, and a physical therapy room and activity center covering nearly 1,800 square feet combined and are fully equipped for physical and occupational therapy.

COCC originally opened as a 90-bed skilled nursing facility with 10 rest home beds.  To meet demand, the 10 rest home beds were converted to nursing home beds shortly after opening.  In 1994, 16 additional nursing home beds were added.  At that time 90 beds were dually certified for Medicare and Medicaid eligible residents and 26 beds were deemed licensed only.  As of April 1, 2006 all 116 beds have become dually certified by the Ohio Department of Health.

Quality is Key
At its most recent annual survey by the Department of Health, COCC was found to be deficiency free in the critical areas of nursing and quality of care.  In its history there have been several annual surveys which were totally deficiency free.  Moreover, Century Oak Care Center has never experienced or had imposed upon it a civil or monetary penalty.

Visitors, both for residents and those who would like to simply view the facility, are welcome 24 hours a day, 7 days a week.  Tours for visitors can be given by experienced staff members any time day or night.
